First Aid Examinations for Members only..

Dear members,

Pls be informed that our first aid examination for members will be held with the following details:

Date: 30th May 2009

Venue: SMK Seafield

Time: 8.00a.m.

Dress code: Half uniform

For members whose ID card has expired or those who are advancing from cadet to adult status, pls prepare 2 IC-sized photos, 2 photocopies of your IC as well as complete the "Butir-butir Ahli" form attached above. Pls make sure that you complete the form in the sheet labelled "KSTS" only and not other areas.

For those whose ID has expired, pls hand in your ID tag as well.

For those who would like to take the exam, kindly confirm your attendance with me or any of our AJKs by this coming Wednesday.

For your info, first aid examination for recruits will be held on 20th June 2009.

p/s AJKs, pls photostat the butir-butir ahli form to be distributed to those who require it and collect it together with the photos, ICs and expired IDs. Deadline for submission to me is this coming Thursday. Pls make sure that ALL details are accurate and in accordance with the details in the members' IC. NO liquid paper allowed, forms must be completed in BLACK ink and must be neat. You the know the rules =)
